At any time, if you or a family member has questions or needs help, call us first. Our team of clinical experts and after hours nursing team are always available to answer your questions and support your needs. We may be able to help you avoid an unnecessary visit to the emergency room.  If you are experiencing a life-threatening emergency, please call 911 immediately.

Remote video visits

Video visits give patients the option to have a face-to-face virtual “visit” with their clinician and the ability to keep their caregiver involved in the communication process.

Our secure channels give patients more flexibility in talking to their nurse, screen-to-screen.

This technology is also about empowering the patient to become engaged in their recovery, and accomplishes this by enabling clinicians to assign condition-specific education to patients directly and monitor their engagement.

Telehealth

Our state-of-the-art technology enables patients to become active in their recovery and have access to their care team at the touch of a screen. It’s an easy-to-use, modern solution that has been proven to help patients stay on their path to recovery and out of the emergency room.

Patients can take their daily vital signs (such as blood pressure and heart rate) and enter this information in the secure app on a tablet or smartphone. Patients will have a few follow-up questions to answer to help the nurse reviewing the information accurately assess their results. Our nurses will call to discuss any concerns and keep your physician up-to-date on your status.

Medication management

Our comprehensive medication management program allows patients to have medications delivered straight to their home. Medications are organized and packaged to help patients manage their medication, adhere to their schedule and alleviate challenges or barriers to receiving prescribed medications.

This service provides a safer, more effective way to help ensure patients have the medication they need and are taking it the way they should. It’s one more way to keep patients safer and healthier in their own homes.

Whether you are looking into home care for the first time, curious about services or exploring costs, we can provide clear answers. Below, we have addressed some of the most commonly asked questions about home care, how it works and what to expect. 

Home care includes a wide array of health care services to help recuperating, chronically ill or disabled patients at home. Home health care can be provided as needed and focuses on treating patients in a familiar environment and reducing return trips to the hospital.

Medicare, Medicaid or private insurance often covers the cost of home health care services when ordered by a physician. Our staff will determine if your home care needs are covered and will complete all the necessary paperwork.

Our goal is to assist a patient’s return to their maximum level of functioning, as well as provide available services to help patients stay safely in their home.  A home care team member will call you to schedule a time to visit you in your home. During this first visit, a physical and environmental assessment will be performed, including a complete medication summary. Once the assessment is complete, we will discuss the options available, and with your consent, call your physician to obtain your home health orders. All patients and families have the right to decide which type of medical care is received and we will work with you and your physicians to develop the best plan of care to meet your individual needs.

The number of home visits a patient needs is based upon the specific skilled needs of each individual patient and family. After the initial assessment is completed in your home, your physician, your home care clinician, alongside both you and your family, will work together to develop a coordinated plan of care. This plan of care will guide us in providing the home health services that will support you in reaching your goals.

It is widely established that among the formal post-acute care settings, home health care is generally the most cost-effective solution. The average cost of a hospital stay is about $2,500 per day while the average cost for home care services is about $45 a day.
