Deaconess Health System and CHI Health at Home Advancing Care Together
Milford, OH – November 2, 2019 – Deaconess Health System and CHI Health at Home, one of the largest home health care systems in the country, have formed a new joint-venture partnership to provide home-based health care solutions throughout Southern Indiana.
Deaconess Health System, a trusted provider with a strong community commitment delivering a comprehensive array of exceptional care directly to patients, has formed this new partnership with CHI Health at Home to enhance their services, assure a stronger and more versatile organization, and to expand its home health care service offerings and value-based solutions under the name of Deaconess Health at Home.
With the launch of the new partnership, Deaconess Home Health integrated CHI Health at Home’s state of the art electronic medical record system, Homecare Homebase, for improved efficiencies and outcomes. Deaconess Home Health will align clinical care programs to support CHI Health at Home’s successful chronic disease management programs that have helped position 79% of their home care locations to achieve a 4 Star rating or higher, in a pursuit of clinical excellence.
