We know our patients have many choices for continuing care after they leave the hospital. Choosing the right setting for your loved one can be difficult. That’s why we have created partnerships with facilities and agencies in our communities that share our strong desire to ensure a smooth transition, effective communication and continued quality care after an acute hospital stay.

About the Continuing Care Network

The Continuing Care Network (CCN) is a collaborative partnership among Virginia Mason Franciscan Health including Franciscan Medical Group, Franciscan Hospice and Palliative Care, Regional Hospital - a long-term acute care hospital (LTACH), and St. Joseph Inpatient Rehabilitation Program; Northwest Physicians Network; community physicians; and select local skilled nursing facilities and home health agencies. Our goal is to provide the best possible health care options for our patients who need continuing care after they leave the hospital.
