Our Accreditations

As part of our mission to deliver health care and support services in a way that a caring family provides, we are committed to clinical excellence and quality care to every patient we touch. It is our goal is to always go above and beyond when it comes to your care or the care of your loved ones.

When searching for a home health provider, it’s important to look for an organization that has received national accreditation for its services, which consists of a professional peer review administered by private, non-profit organizations who establish standards beyond what is required by the state or federal government.

We provide quality care to help our patients improve their own quality of life. And we do it in the place they want to be – their home.

CHI Health at Home works with the Accreditation Commission for Health Care (ACHC) for quality assurance and accreditation.  

An independent, private, not-for-profit organization, ACHC was established by home care and health care industry providers with the goal to provide an accreditation program that helps organizations improve operations and the quality of patient care and services they provide. ACHC has developed comprehensive standards and guidelines and partners with CHI Health at Home to provide the tools and resources needed to deliver the best patient care. 

While not a requirement for our home care and hospice service lines, all home infusion and home respiratory care services are required to be accredited. CHI Health at Home is committed to working with and implementing ACHC's standards across all service lines to ensure our patient care and services are provided in a safe manner.

Our Awards

As a result of our hard work, the ABILITY Network and DecisionHealth recognized 16 CHI Health at Home locations as HomeCare Elite in 2017.

HomeCare Elite consists of the top 25 percent of all Medicare-certified agencies and highlights the top 100 and 500 agencies overall. Out of 9,064 agencies considered, 2,268 are recognized as “elite.” Winners are ranked by an analysis of publicly available performance measures in the following areas:

  • Quality outcome
  • Best practices (process measure) implementation
  • Patient experience (Home Health CAHPS® or HHCAHPS)
  • Quality improvement and consistency
  • Financial performance
